Goal: The Carrera Adolescent Pregnancy Prevention Program's goal is to provide comprehensive youth development services and reduce teen pregnancy among economically disadvantaged teenagers.

Impact: Pregnancy prevention programs can work successfully among females when started early in adolescence and when male counterparts are also educated appropriately on condom-use and delayed sexual actively onset.

Goal: CAPABLE is a 5-month structured home visit program delivered by an occupational therapist (OT), a registered nurse (RN), and a handyman to improve daily function in older adults and to lower the monthly average Medicaid expenditure and likelihood of costly healthcare services.

Impact: This study demonstrates that home visit programs can improve the daily quality of life in aging adults. Additionally, they can lead to a reduction in Medicaid expenditures via lower inpatient costs and lower long-term care costs.

Goal: The goal of the Environmental Health Leadership Training is to inform and empower the predominately low income people of three urban communities in Northern Manhattan (Central Harlem, West Harlem, and Washington Heights) to improve their capacity to organize for community environmental health and justice in New York City. The long term goal of these efforts is to help intervene and reduce exposure to environmental toxicants which are adversely affecting the health of disadvantaged, medically underserved, predominantly African American and Latino populations in Northern Manhattan.

Impact: Insights proves that tailored cognitive/behavioral minimal self-help interventions hold promise as HIV/STD prevention strategies for diverse populations of young at-risk women.

Goal: The John Hopkins Community Health Partnership's (J-CHiP) goal is to improve care coordination with Medicare and Medicaid beneficiaries.

Impact: The John Hopkins Community Health Partnership participants saw lower spending and improved health outcomes in regards to hospital admissions, re-admissions, and emergency department visits.

Goal: The goal of this program is to improve colorectal cancer screening rates among older adults.

Impact: Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

Goal: The PHLAME Study had four primary goals: (1) Increase physical activity to 30 minutes each day; (2) Reduce percent calories from fat to less than 30%; (3) Increase servings of fruits and vegetables to at least 5 per day; and (4) Improve energy balance and normalize body fat.
