Goal: The mission of the Greene Community Health Foundation is to 1. help assist underserved GCCHD clients with funding for medical needs, 2. find avenues of funding for clients, 3. develop partnerships with providers for reduced charges for clients, 4. develop community partnerships to strengthen programs and projects at GCCHD, and 5. develop state and national partnerships to strengthen the programs and projects at GCCHD.

Goal: The primary goal of the intervention was to reduce overweight and obesity prevalence among middle school students. Ancillary goals were to improve BMI and fasting insulin values, increase water consumption, reduce consumption of beverages with added sugar, increase healthy food choices, improve self-monitoring, and increase exercise time among sixth, seventh, and eighth grade students.

Impact: School-based programs that aim to address childhood obesity and adiposity may reduce individuals' risk of developing childhood-onset of non-insulin dependent diabetes mellitus.

Goal: The Incredible Years® Parents, Teachers, and Children Training Series has two long-range goals. The first goal is to develop comprehensive treatment programs for young children with early onset conduct problems. The second goal is the development of cost-effective, community-based, universal prevention programs that all families and teachers of young children can use to promote social competence and to prevent children from developing conduct problems in the first place.

Impact: Studies have shown that children who participate in the programs demonstrate significant improvements in school readiness, emotional regulation, and social skills, as well as reductions in behavior problems in the classroom.

Goal: The end goal of The Living Room is to reduce recidivism to the hospital and pair those in need with the right resources to help them be able to handle their mental health crises and move away from using The Living Room. Additionally, a main goal of The Living Room is matching those in need with a particular peer specialist to create a lasting connection.

Impact: Connects individuals experiencing mental health illness or crises to peer specialists which help reduce recidivism to the emergency room or hospital, help manage future crises, and provide a connection to continued non-clinical support and resources.

Goal: To reduce the number of individuals with mental illness who use the ER, to help individuals achieve long-term recovery, and to develop a community peer-based support network.

Impact: The Living Room is a comfortable, non-clinical space that offers an alternative to hospital emergency rooms. The program in LaGrange, Illinois hopes to decrease unnecessary hospitalizations.

Goal: GAIN is part of a large-scale, welfare-to-work initiative program operating in every county in California. In L.A. County, the initiative is under the supervision of the Department of Public Social Services. It helps local businesses and employers find and hire quality workers who seek meaningful employment. Prospective workers are participants in the state welfare programs known as California Work Opportunities and Responsibility to Kids (CalWORKS) or General Relief Opportunities for Work (GROW).

Goal: The Louisiana Recovery Authority is working with Governor Blanco to plan for Louisiana's future, coordinate across jurisdictions, support community recovery and resurgence, and ensure integrity and effectiveness. Working in collaboration with local, state and federal agencies, the authority is addressing short-term recovery needs while simultaneously guiding the long-term planning process.

Goal: The Memphis Enviromental Court was designed to serve as the chief vehicle for enforcing a wide range of laws relating to the health and quality of life of its residents, and to give new meaning to the term "expedient justice" by its ability to respond in a quick and consistent manner. In 1991 the Tennessee State Legislature created the Shelby County Environmental Court, and expanded the new court's powers to include the "injunctive authority" to mandate that defendants comply with the environmental codes.

Goal: The aim of the Montana Model is to provide health care to the 11 percent of the county citizens living below the federal poverty level and the many more living in near poverty, while simultaneously providing a medical clinic with diversity and complexity for family practice residents to obtain enriched training with the public health perspective.
